Should the camper need over-the-counter medication (headache relief medicine, cough drops, decongestants, etc.), I give my consent to Temple Shalom to dispense products or their generic equivalent below without having to consult me, as designated here.
At Temple Shalom Day Camp, the safety and well-being of our campers are our top priorities. To create a joyful, wonder-filled, safe, fun, and nurturing environment, we ask all families to review and agree to the following safety procedures:
Authorized Pick-ups: Only individuals listed on your child’s registration form may pick them up. ID may be required. Late Pick-up: Families must notify the camp if running late. Late fees may apply. Check-in & Check-out: A designated adult must sign in and out each camper daily. Healthy Attendance: Please keep your child home if they show signs of illness (fever, vomiting, contagious conditions, etc.). Hydration & Sun Protection: Send a refillable water bottle, sunscreen, and a hat with your child each day. Shoes & Clothing: Campers should wear closed-toe shoes for outdoor activities and comfortable clothing that can get messy. Allergies & Medical Needs: Inform camp staff of any allergies, medications, or medical needs before camp begins.
Camper Behavior & Respect
Kindness & Inclusion: Campers are expected to treat others with respect and kindness. Listening to Staff: Campers must follow instructions for their own safety and the safety of others. No Unsafe Items: Campers may not bring sharp objects, electronic devices, or personal toys unless approved. Use of supplies: Campers must follow all safety instructions. Garden Tools & Nature Play: Campers will use garden tools safely and avoid picking or touching unknown plants.
By SUBMITTING THIS FORM, I acknowledge that I have read and agree to the Temple Shalom Day Camp safety procedures and will review them with my child before camp.
EMERGENCY AUTHORIZATION
In the event an injury or illness is life threatening, I authorize TEMPLE SHALOM DAY CAMP to summon and all professional emergency personnel to transport and treat my child. I give permission personnel selected by TEMPLE SHALOM DAY CAMP to provide and arrange care and transport, and to issue consent for any x-ray, anesthetic, blood transfusion, medication or other medical evaluation, treatment or care deemed advisable by, and to be under the general supervision of, a licenses medical professional. I agree to assume financial responsibility for all expenses of such care. I give permission for this form to be copied for use out of camp.
ATTESTATION I attest that I have answered all of the questions truthfully and to the best of my knowledge. Should any of this information change prior to the participants' arrival at camp, I will contact the camp director immediately with changes or additional information. In order to help make this experience a success for the participant, it is critical that you provide as much information as possible. Failure to disclose pertinent health information may result in the participant being unable to participate in camp activities or early dismissal from camp.
Privacy and Use of Images Families may be photographed or videotaped as they participate in camp activities, or they may be recorded in Temple Shalom video stream programs. On occasion, these images, and the identity of the persons depicted, may be used by Temple Shalom for educational, instructional, development and/or informational purposes in print, internet and digital media, including in news releases (e.g., Texas Jewish Post) and/or printed publications or materials, electronic publications, or websites. Temple Shalom will not sell images of Temple Shalom members or non-members or their families to third parties, and it will not use their images for promotional or advertising purposes that do not concern Temple Shalom and its programs.
CONSENT TO ELECTRONIC TRANSACTION
I hereby agree to conduct this transaction by electronic means pursuant to the Uniform Electronic Transactions Act, Texas Business and Commerce Code Chapter 322.
